Fiberoptic Specialties Inc. LS86/110 150 W Fiber Optic Lite Source Illuminator
This illuminator has some scratches scuffs and dings but it functions fine.  It also has some putty type substance on it that looks like it would scrape off with a proper scraper.  It lights up fine and the light intensity increases when I turn the dial up.  Everything that is included is shown in the pictures.
